While researching lesson learned about the Deepwater Horizon Spill I came across the following Texas Refinery Explosion speech by BP Senior Group Vice President for Safety & Operations John Mogford. “This was a preventable incident,” he said. “It should be seen as a process failure, a cultural failure and a management failure.”
